Dear PCD Dad,

PCD Smiles ships to people with PCD of all ages who are going through or just arriving home from a hospitalization. However at the present time, Smiles’ boxes are currently only available for PCDers who reside in the United States and Canada. Please go to PCD Smiles’ website (http://www.pcdsmiles.com/) and click the “Request Cheer Package” link located in the top middle of the page. Please be aware that if your daughter’s discharge date falls within five business days, or might fall within five business days then PCD Smiles will need to ship the cheer package to her home to ensure it gets to her. Unfortunately hospitals will not forward boxes to a patient after discharge, they only forward cards and letters. Please include her complete home address on the form, this helps PCD Smiles avoid delays in shipping the cheer box. PCD Smiles confidentiality policies will ensure that the patient’s and the requester’s information is keep private and confidential.
