Symptoms of PCD start very early in life, usually during the first year, and include:

■■ Neonatal respiratory distress (trouble breathing shortly after birth) in full-term babies, usually requiring oxygen therapy, and often lasting days to weeks
■■ Daily, year-round, nasal congestion
■■ Daily, year-round, wet (mucus-producing) cough
■■ Chronic middle ear fluid and ear infections that can lead to
hearing loss or speech difficulties
■■ Chronic sinus infections
■■ “Sidedness”differences(situsinversustotalisorsitus
ambiguus/heterotaxy), including congenital heart
defects
■■ Male infertility (inability to make a woman pregnant),
since sperm tails use the same genes as motile cilia
■■ Reduced fertility in women (inability to get pregnant)

Most medical textbooks still propagate the myth that PCD is a mild disease associated with a normal lifespan. We have now learned through large population-based studies and registries that there is a wide spectrum of severity of lung disease in PCD, from relatively normal lung function associated with a normal lifespan to progressive widespread bronchiectasis with need for lung transplantation in early adulthood (1). Although the very limited longitudinal data (2) available suggest that pulmonary function may be stable over many years when appropriate therapy is instituted, the largest cross-sectional study (3) to date, which evaluated over 100 North American patients with PCD, suggests that there is a definite decline in lung function with age, with many adult patients having lung function within the transplant range. ~ Primary ciliary dyskinesia: Myths and realities; Sharon D. Today’s featured musicians are David Goldfarb - Tuba, Nina Guerrero - Piano with their submission titled! “Two Vocalises by Concone”.

This episode of Family Music is posted in observance of PCD Awareness Month. A big reason that I play brass instruments is that I have Primary Ciliary Dyskinesia (PCD), a genetic condition that affects the cilia, which are hairlike structures in the lining of the lungs and other organs that, among other things, move mucus and whatever dust and germs we breathe in out of the lungs. Since my cilia can't do that effectively, I've always had a cough that sounds like some serious infection, but really it's normal for me, and it's not communicable to others. Folks with PCD stimulate the movement of fluid from the lungs by using medications and physical activity like vigorous exercise, percussion vests, and flutter valve breathing devices, but for me nothing is more effective and motivating than playing the trombone or the tuba for a few hours. Before COVID-19, I was rehearsing or performing five or six nights a week. For now, I stay in practice and record with my family at home and in virtual ensembles with other musicians, and look forward to a time when we can all perform in public again. ~ David A. Goldfarb
