“Imaging studies show that lung disease in PCD begins in infancy or early childhood, as revealed on high-resolution chest CT. These findings manifest as subsegmental atelectasis, peribronchial thickening, mucus plugging, and/or evidence of air trapping and ground-glass opacity. Bronchiectasis can occur during infancy, and inevitably develops as disease progresses (21). Approximately 50–75% of older pediatric patients and nearly all adults with PCD have bronchiectasis with predilection for middle lobe, lingula, and basal segments (34–36). Similar to CF, chest CT scanning is more accurate to assess early PCD lung disease compared with spirometric indices (38). Chest high field 3T magnetic resonance imaging appears to be as effective as high-resolution chest CT in assessing the extent and severity of lung abnormalities in PCD, making this radiation-free imaging technique an intriguing option for longitudinal assessment and research studies (43).” ~ American Thoracic Society

Today’s featured artist is Jason Boehmer with her submission titled “Metamorphosis”.

“Metamorphosis” is a powerful visual representation of the difficult journey to lung transplant by a patient with primary ciliary dyskinesia (PCD). The artwork depicts a lady with PCD in her anguish as she casts off her old damaged lungs and flings them with her hands. As her old tattered lungs soar away from her hands they transform into a butterfly, a symbol of hope and transformation. While lung transplants in primary ciliary dyskinesia care are not curative, transplants do provide the patient with some ease of breathing, for possibly the very first time in their lives. However the path to transplant is a long and arduous one, full of its own inherent issues and struggles. Transplants journeys are tough.

The PCD Artist Project is a collaborative effort by artist to raise awareness for PCD or primary ciliary dyskinesia and support the PCD Smiles program. Each artist who submitted art work to the project either has PCD or has a PCD family member.

“Early manifestations of PCD occur in the neonatal period, as over 80% of full-term neonates with PCD have respiratory distress. These infants have tachypnea, increased work of breathing, and frequently require supplemental oxygen for a few hours to weeks (18–20). These infants are often diagnosed with transient tachypnea of the newborn or neonatal pneumonia. This neonatal presentation implies that normal ciliary function plays a critical role in the clearance of fetal lung fluid, although the mechanism is unknown. The presence of unexplained respiratory distress and radiographic abnormalities (e.g., atelectasis and/or increased perihilar/interstitial markings), along with supplemental oxygen requirements in a full-term infant, should raise suspicion for PCD, particularly in neonates with situs inversus.” ~ American Thoracic Society

“A Part Time Job” is available in both Italian and English versions.

“PCD is a progressive disorder, meaning it will get worse over time, for which no cure currently exists. For most patients, it is thankfully very slowly progressive. There have been limited clinical trials of therapies conducted for PCD, and there are no ‘approved’ treatments at this time. However, because PCD shares some features with cystic fibrosis (CF), a more widely-known lung disorder for which there are many tested and proven therapies, much of the current PCD treatment protocol mirrors that of CF. While CF-type disease management may benefit those affected by PCD as the primary focus in both disorders is to prevent disease progression through aggressive treatment of infections, it is important to remember that these disorders are genetically distinct and there is no actually evidence to suggest that what works in CF is the best option for PCD. More PCD-specific research is desperately needed to determine best practices for PCD.” ~ American PCD Foundation
